
To address the technical aspects of standardizing surgical care, the Alliance/American College of Surgeons (ACS) Clinical Research Program (CRP), a program of the Alliance for Clinical Trials in Oncology and the College, formed the Cancer Care Standards Development Committee in 2013. Since then, the committee, led by Matthew H. G. Katz, MD, FACS, Chair, and Nirmal Veeramachaneni, MD, FACS, Vice-Chair, has brought together more than 400 surgical oncologists to publish Operative Standards for Cancer Surgery, Volumes I and II. These manuals were developed in response to the need for a standardized description of “minimum standards” for various cancer operations and are valuable tools for the practicing surgical oncologist.
Operative Standards for Cancer Surgery Volume I, published in 2015, details the critical elements of surgical procedures for breast, colon, lung, and pancreatic cancer. Volume II, which will be published in mid-2018, will include operations for melanoma and gastric, esophageal, rectal, and thyroid cancers.
The manuals provide concrete suggestions on the effective conduct of operations and detailed information regarding the oncologic principles, avoidable pitfalls, and quality of the evidence on which these recommendations are based. The manuals focus on the decisions that surgeons make from incision to closure, with recommendations that are based on the strongest available evidence.

Because randomized trials have yet to address all of the components of operations within each disease site, it was important to draw on the experience and consensus of the experts writing the individual chapters. Identifying the lack of evidence on certain topics has been an unintended consequence of developing the manuals and has galvanized the resolve of the authors to establish standards where none exist.
